To orderly and efficiently process your Claim Form, please print or type clearly all of the information requested on the form. Incomplete claim forms may be returned to you as insufficient. Completed originally signed claim forms may be mailed or personally delivered to:
You will receive a letter from the Risk Management Office indicating your claim has been received and is being investigated. You will receive an explanation of the investigation results within 45 days in most instances.
